Job Description:

the comfort of their homes. As a Licensed Practical Nurse (LPN),
you will work closely with RNs and physicians to carry out patient
care plans, administer medications, and monitor patient progress to
achieve the best health outcomes. Your role is pivotal in educating
patients and their families about in-home treatments and preventive
measures. Our commitment is not just to heal but to enhance lives
through attentive healthcare tailored to individual needs.
Administer medications and treatments as prescribed by physicians.
Monitor and record the patient's condition, reporting any changes
to registered nurses and doctors. Educate patients and their
families on health care, including medication, treatment, and
preventive care protocols. Provide basic patient care inclusive of
bathing, feeding, and dressing while ensuring comfort at all times.
Participate in the evaluation and review of care plans for each
patient. Maintain patient confidentiality and adhere to HIPAA laws.
Proficiency in administering medications and understanding care
protocols. Strong interpersonal and communication skills.
Detail-oriented with a keen ability to observe and report changes
in patient condition. Basic computer skills for documenting patient
records. Ability to work both independently and collaboratively in
