care. Responsibilities 1. Develop and implement clinical plans of
care for adult patients with chronic and terminal illnesses. 2.
Perform patient assessment, history and physical; recommendations
on aggressive symptom-management. 3. Develop plans to meet patient
and family physical and psychosocial needs 4. Monitor patient
progress; update the Team Director and team members; consult
regarding discharge planning; and patient/family teaching 5.
Maintain patient medical records according to established standards
and documents in a systematic, accurate and succinct format 6.
Maintain communication with the supervisory physician(s) and
attending physician to review patient records and care plans on a
routine basis 7. Refer patients to Physician or to specialized
healthcare resources for consultation or treatment as appropriate
8. Initiate communication with attending physicians, other hospice
staff members and other agencies as needed to coordinate optimal
care and use of resources for the patient/family. 9. Provide
patient/family education and counseling including instructing the
family and/or patient caregiver on: How to administer medication
and recognize side effects. How to perform personal care and oral
hygiene. How to assist in lifting, moving, ambulation and exercise.
How to prepare for the events preceding death 10. May provide
appropriate support at time of death 11. Assist in making home
health aide assignments, prepare written instructions for the aide
and supervise the aide in the home 12. Participate in continuing
education and/or continuing medical education as required by
licensing body 13. Prescribe medication as permitted by the state
and/or the state Board of Nursing 14. Serve as a resource to
hospital, facility staff and/or physicians on palliative care
issues. 15. Participate in hospice orientation and ongoing
education programs 16. Supervise and delegate to members of the
care team as appropriate 17. Other duties as assigned Pre-Hospice
1. Face-to-face clinical and needs assessment at intake, preferably
in the patient's home setting, inclusive of the patient's medical,
emotional, spiritual, social, and practical needs 2. Face-to-face
reassessment done when needed and at least every 60 days Plan of
care development based on these assessments and incorporating the
patient's and family's priorities 3. Conduct follow-up with the
patient and family, through telemedicine, telephone calls, writing
or in-person, between assessment visits, as needed 4. Consult and
involve appropriate member of the Pre-Hospice team to provide
follow up whenever possible and as appropriate 5. Complete a
hand-off of the patient at time of discharge, whether to primary
care, hospice or any other discharge destination 6. Clinical
supervision of the care plan including all medicine orders 7.
Collaborate with the rest of the Pre-Hospice team including
alerting the team to changes and newly identified patient needs as
they arise Qualifications 1. Master's degree, required 2. Current
and unencumbered advanced practice nursing license in state(s)
practicing 3. Current and valid CPR certification required 4.
Certified by a nationally recognized advanced nursing practice
organization 5. Minimum one-year experience, required in California
6. Previous experience in hospice, home health, general medical
surgical or patient population specific (i.e.: geriatrics,
pediatric, etc.) nursing 7. Demonstrates familiarity with policies
of the agency and state(s) and federal regulations pertaining to
nursing services. 8. Demonstrated expertise in pain/symptom
management and advanced care planning 9. Must have reliable
automobile, valid driver's license, and the minimum state required
liability auto insurance 10. Commitment to AccentCare's philosophy
